At CareClan, we are committed to delivering high-quality care and ensuring the best possible outcomes for those using our services. When an unintended or unexpected incident causes harm, we follow the Duty of Candour procedure. This means we inform those affected (or their families/carers) to ensure they understand what has happened and provide an apology. We also monitor and review our learning from these incidents to improve our quality of care.

An important part of this duty is to provide an annual report showing any learning from Duty of Candour incidents in our service. This report describes how our care service has operated the Duty of Candour between 1 April 2023 and 31 March 2024. We hope you find this report useful.

 

How many incidents happened to which the Duty of Candour applies?

Since CareClan Solutions Limited was established on 12 March 2024, we have not been operational for a full year and therefore, there have been no incidents to which the Duty of Candour applied during this period.

 

Information about our policies and procedures

When an incident triggers the Duty of Candour, our staff report this to Chris McLean, who has the responsibility for ensuring that the Duty of Candour procedure is followed. The manager records the incident and reports it to the relevant authorities as necessary. When an incident occurs, the manager and staff participate in a learning review. This review allows everyone involved to understand what happened and identify changes and improvements for the future.

 

Training and Support

Duty of Candour training is available to all employees. We recognise that serious mistakes can be distressing for our staff, as well as for those using our services and their families. We provide welfare support for employees affected by a Duty of Candour incident. For those using our services and their families affected by the Duty of Candour, we have arrangements in place to provide necessary welfare support.
